         of acceptance.

·        Written proof of age and social security number must be submitted.

·        All entries must be received by Friday July 30^th.

·        Please mail completed form to the: Steuben County Fair, 15 East Washington Street, Bath, New York 14810

         and mark the envelope “Talent Contest” or drop off the completed entry at the Fair Office.

·        For Multiple entries please photocopy the forms as needed.
